We'd like to get to know you and your dog(s) so we can hit the ground running. If you are new to Nysa Hill, please fill out the brief questions below, then press "SEND" at the bottom so that we can receive your information.
We spend our days and nights working with the dogs visiting. Please email us with any questions through: NysaHill@gmail.com
Clients should have texting and/or remote email capabilities for updates, status reports and client communications. Because our priority is working the dogs, we ask that new queries be made via email, not by phone. Emails and texts will also receive the quickest response.
PLEASE NOTE: We do not accept same day, unscheduled "drop-ins" for new dogs; each dog must be temperament evaluated, and medical records provided, prior to the first visit. Thanks!

| Your Best Friend's Checklist For Arrival at Nysa Hill Prior to, or the day of your initial arrival, we will ask you for the following items:
VETERINARY STATUS
_____ A copy of your latest vaccination records for the dog arriving. Must be current on the following vaccines: DISTEMPER, HEPATITIS, LEPTOSPIROSIS, PARAINFLUENZA, ADENOVIRUS, PARVO (These are often given in one combination vaccine), plus RABIES and BORTADELLA. Dogs without proof of current vaccinations cannot visit or stay at Nysa Hill. We offer dog travel service to and from vets with advance notice.

| _____ Your dog must be current on anti-flea and tick medications, and be flea and tick free upon arrival. We offer anti-flea and tick medications if you would like us to administer your newest application. The name of the anti-flea/tick medication you are current using:

_____ A list of phone numbers, locations, instructions on how to reach you at all times. Full itineraries even better.
TO BRING:
_____ Your dog's collar and name tag. If you do not have a current name tag, you can purchase one from Nysa Hill.

| _____ Containers (we recommend plastic food containers or Hefty baggies) with your dog's regular canned food and/or dry food. Please label it with your dog's name, and note the type of food it is (e.g., Royal Canin Special 25). Please bring only enough for the dog's stay. Nysa Hill can also feed your dog, however we recommend using your own food for at least some of the stay as changes in diet can create upset stomach, diarrhea, constipation or cramps.

_____ Bedding or blankets for your dog. We will provide them.
_______ Dog bowls, dishes, glassware or porcelain of any kind. Nysa Hill will provide all bowls and dishes.
_______ Spike, choke chain or harness collars (training exceptions upon approval).
